Acknowledgment of Risk


  • I understand that participating in water activities and any other activities provided by Unique Parties by Katie, including but not limited to foam parties, slip and slides, inflatables, games, and other recreational entertainment, involves inherent risks, including but not limited to:Slipping, tripping, or falling
  • 
  • Collisions with other participants
  • Sprains, strains, bruises, cuts, broken bones, or other injuries
  • Exposure to wet surfaces
  • Allergic reactions or skin irritation
  • Other risks that may result from participation in these activities

I voluntarily choose to participate and accept all risks associated with these activities.


Release of Liability


In consideration for being permitted to participate, I, on behalf of myself, my child(ren), my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless Unique Parties by Katie, its owners, employees, contractors, entertainers, volunteers, and affiliates from any and all claims, demands, causes of action, damages, injuries, losses, liabilities, costs, or expenses arising out of or related to participation in the event, including personal injury, property damage, or death, except where prohibited by law.


Assumption of Responsibility


I agree that:

  • Participants will follow all instructions provided by Unique Parties by Katie staff.
  • Participants will wear appropriate clothing and footwear for water activities.
  • Participants who are ill, injured, or under the influence of drugs or alcohol will not participate.
  • Children must be supervised by a parent or responsible adult at all times unless otherwise agreed upon.
  • I am responsible for determining whether I or my child is physically capable of participating.

Medical Authorization


In the event of an injury or medical emergency, I authorize Unique Parties by Katie to seek emergency medical treatment if necessary. I understand that Unique Parties by Katie is not responsible for any medical expenses, hospital bills, ambulance fees, or other healthcare costs resulting from participation in the event. I acknowledge that I am solely responsible for any and all medical expenses incurred by myself or my child.


Property Damage


Unique Parties by Katie is not responsible for any lost, stolen, or damaged personal belongings brought to the event.

The event host is responsible for ensuring the activity area is reasonably free from hazards before setup and agrees to notify Unique Parties by Katie of any unsafe conditions.


Weather


I understand that outdoor events may be delayed, modified, or canceled due to unsafe weather conditions at the discretion of Unique Parties by Katie.


Photography


Unless otherwise requested in writing, I grant permission for photographs and videos taken during the event to be used by Unique Parties by Katie for promotional purposes, including social media and marketing materials.
